What appointment are you coming to our clinic for?

Coaching program

Thank you for taking time to book your Coaching Program with us.

At the core of our assessments are three major steps.

  1. A background information review and analysis – See below for steps on how to prepare for your visit
  2. Body Fat Analysis using Biosignature Modulation
  3. Food Sensitivity Tests using electro-dermal screening

How to prepare for your Coaching Appointment:

1. Please be sure to fill out this Client Assessment Form. This form takes approximately 10 minutes, and it required to be filled out prior to your appointment. It is your initial intake form that will help us better prepare for our meeting.

2. Please fill out and bring THREE days worth of written food logs. Download Daily Food Log Sheets here.

3. Preparation for the body fat test:

    • Males – please bring shorts
    • Females – please bring shorts and a sports bra or a 2 piece swimsuit.

Food Sensitivity Test & Menu Plan or Food Sensitivity Test

At the core of our assessments are two major steps.

  2. Food Sensitivity Tests using electro-dermal screening

How to prepare for your Food Sensitivity Test and Appointment:

✓ Please fill out all paperwork 48 hours prior to your appointment. Please be sure to fill out this Client Assessment Form. This form takes approximately 10 mintues, and it required to be filled out prior to your appointment. It is your initial intake form that will help us better prepare for our meeting.
✓ Please fill out and bring THREE days worth of written food logs. Download Daily Food Log Sheets here.
✓ Please drink no less than 3L of water prior to your appointment. The test may NOT work if you are dehydrated.
✓ Please make sure that you eat a meal(s) that has protein, vegetables, and starches (rice or potatoes) prior to your appointment.
✓ Please do not workout the day of your appointment.
✓ Please do not have any caffeine products (this includes coffee and teas).
